Looking to quit smoking? Vaping more effective than patches and gum: study

WASHINGTON — A major new study provides the strongest evidence yet that vaping can help smokers quit cigarettes, with e-cigarettes proving nearly twice as effective as nicotine gums and patches.

The British research, published Wednesday in the New England Journal of Medicine, could influence what doctors tell their patients and shape the debate in the U.S., where the Food and Drug Administration has come under pressure to more tightly regulate the burgeoning industry amid a surge in teenage vaping.

“We know that patients are asking about e-cigarettes and many doctors haven’t been sure what to say,” said Dr. Nancy Rigotti, a tobacco treatment specialist at Harvard Medical School who was not involved in the study. “I think they now have more evidence to endorse e-cigarettes.”

At the same time, Rigotti and other experts cautioned that no vaping products have been approved in the U.S. to help smokers quit.

Smoking is the No. 1 cause of preventable death worldwide, blamed for nearly 6 million deaths a year. Quitting is notoriously difficult, even with decades-old nicotine aids and newer prescription drugs. More than 55 per cent of U.S. smokers try to quit each year, and only about 7 per cent succeed, according to government figures.

Electronic cigarettes, which have been available in the U.S. since about 2007 and have grown into a $6.6 billion-a-year industry, are battery-powered devices that typically heat a flavoured nicotine solution into an inhalable vapour.

Most experts agree the vapour is less harmful than cigarette smoke since it doesn’t contain most of the cancer-causing byproducts of burning tobacco. But there is virtually no research on the long-term effects of the chemicals in the vapour, some of which are toxic.

At the same time, there have been conflicting studies on whether e-cigarettes actually help smokers kick the habit. Last year, an influential panel of U.S. experts concluded there was only “limited evidence” of their effectiveness.

In the new study, researchers tracked nearly 900 middle-age smokers who were randomly assigned to receive either e-cigarettes or nicotine replacement products, including patches, gums and lozenges. After one year, 18 per cent of e-cigarette users were smoke-free, versus 9.9 per cent of those using the other products.

“Anything which helps smokers to avoid heart disease and cancer and lung disease is a good thing, and e-cigarettes can do that,” said Peter Hajek, study co-author and an addiction specialist at Queen Mary University of London.

The study was more rigorous than previous ones, which largely surveyed smokers about e-cigarette use. Participants in this experiment underwent chemical breath testing.

Smokers in the e-cigarette group received a $26 starter kit, while those in the nicotine-replacement group received a three-month supply of the product of their choice, costing about $159. Participants were responsible for buying follow-up supplies.

“If you have a method of helping people with smoking cessation that is both more effective and less costly, that should be of great interest to anyone providing health services,” said Kenneth Warner, a retired University of Michigan public health professor who was not involved in the study.

Several factors may have boosted the results: All the participants were recruited from a government smoking-cessation program and were presumably motivated to quit. They also received four weeks of anti-smoking counselling.

The researchers didn’t test e-cigarettes against new drugs such as Pfizer’s Chantix, which has shown higher rates of success than older nicotine-based treatments.

Funding for the study came from the British government, which has embraced e-cigarettes as a potential tool to combat smoking through state-run health services. Some of the authors have been paid consultants to makers of anti-smoking products.

U.S. health authorities have been more reluctant about backing the products, in part because of the long-term effects are unknown.

“We need more studies about their safety profile, and I don’t think anyone should be changing practice based on one study,” said Belinda Borrelli, a psychologist specializing in smoking cessation at Boston University.

The American Heart Association backed e-cigarettes in 2014 as a last resort to help smokers quit after trying counselling and approved products. The American Cancer Society took a similar position last year.

An editorial accompanying the study and co-written by Borrelli recommended e-cigarettes only after smokers have tried and failed to quit with FDA-approved products. Also, doctors should have a clear timeline for stopping e-cigarette use.

Borrelli noted that after one year, 80 per cent of the e-cigarette users in the study were still using the devices. Nine per cent of the participants in the other group were still using gums and other nicotine-replacement products.

No vaping company has announced plans to seek FDA approval of their products as a quit-smoking aid. Winning such an endorsement would require large studies that can take years and cost millions of dollars.

The FDA has largely taken a hands-off approach toward vaping. It has not scientifically reviewed any of the e-cigarettes on the market and has put off some key regulations until 2022. FDA Commissioner Scott Gottlieb has said he doesn’t want to over-regulate an emerging industry that could provide a safer option for adult smokers.

The delay has come under intense criticism amid an explosion in teenage vaping, driven chiefly by devices like Juul, which resembles a flash drive. Federal law prohibits sales to those under 18, but 1 in 5 high school students reported vaping last year, according to a government survey. It showed teenage use surged 78 per cent between 2017 and 2018.

Matthew Myers of the Campaign for Tobacco-Free Kids noted that the British study used so-called tank-based e-cigarettes, which allow users to customize their flavours and nicotine levels. Those devices have largely been overtaken in the U.S. by Juul and similar devices that have prefilled nicotine cartridges, or pods. Any benefit of e-cigarettes depends on the individual product and how it is used, he said.

“It is a fundamental mistake to think that all e-cigarettes are alike,” Myers said. “And in the absence of FDA regulation, a consumer has no way of knowing if the product they are using has the potential to help them or not.”

Myers’ group is one of several anti-smoking organizations suing the FDA to immediately begin reviewing e-cigarettes.

Ian Armitage was skeptical about e-cigarettes as a way to stop smoking, saying he tried vaping several years ago but gave it up after experiencing twitching and shakes from nicotine withdrawal.

“I tried it for a whole month, but it just wasn’t doing it for me,” said Armitage, an audio-visual technician in Washington. “I still wanted a cigarette afterward.”

Armitage, who has smoked for 15 years, said he also tried nicotine patches but found they irritated his skin.

Researchers warn about the severe psychological distress caused by eating junk food

Scientists link junk food to poor mental health

Published in the journal International Journal of Food Sciences and Nutrition in early 2019, a study from Loma Linda University scientists finds a link between poor diet and poor mental health. Even after adjusting for external factors such as gender, age, education and income level, the association between junk food intake and mental illness remained.

Study leader Jim E. Banta, Ph.D., MPH, an associate professor at the school, says that their conclusions support the findings of previous research. To conduct their study, Banta and his team looked at data from the California Health Interview Survey (CHIS). The scientists used 240,000 phone surveys conducted by CHIS between 2005 and 2015, and included data on socio-demographics, health status and health behaviors.

“This and other studies like it could have big implications for treatments in behavorial medicine,” Banta said of the findings.

“Perhaps the time has come for us to take a closer look at the role of diet in mental health, because it could be that healthy diet choices contribute to mental health. More research is needed before we can answer definitively, but the evidence seems to be pointing in that direction,” he added.

The fact that scientists in the 21st century are only now just beginning to even consider the possibility of a relationship between nutrition and mental health is truly disturbing. The Western diet impairs pattern separation

In this study, researchers investigated the utility of spatial separation – a behavioral process associated with the hippocampus – in the assessment of dietary interventions and the behavioral effects of the transgenerational administration of a Western diet on pattern separation. Pattern separation is the process of keeping items distinct in memory and is mediated by the hippocampus. Previous studies have suggested that there is a relationship between hippocampal function and diet quality in both humans and animals.

To examine the association between them, the researchers used rats, feeding over seven generations a diet containing increased amounts of sugar and saturated fatty acids, reduced levels of polyunsaturated fatty acids, and an increased ratio of omega-6/omega-3 fatty acids. This diet composition is characteristic of a diet known as the Western diet. The researchers administered it transgenerationally because previous studies have shown that interventional diets need to be implemented over several generations to induce behavioral effects.

They compared the spatial pattern separation (or local discrimination) performance of these animals with that of rats fed a standard diet. For the test, they presented the rats two locations and allowed them to learn across trials to respond to the correct location. During spatial discrimination training, they designated a location as the correct one and rewarded the rats if they touched the correct location. They reversed the correct and incorrect locations every time the rats successfully got the correct ones nine times out of 10 trials.

The researchers found a separation-dependent difference between the standard and Western diet groups in the number of discriminations performed in the pattern separation task. The rats fed with a Western diet performed fewer discriminations. Rats with lesions in the dorsal hippocampus showed impaired pattern separation when the locations were close together but not when they were far apart. The researchers associated this impairment with hippocampal dysfunctioning. Their results align with previous studies which demonstrated that consumption of a Western diet impaired cognitive functions, damaged brain regions, and contributed to the occurrence of neurodegenerative diseases. Their results confirmed that pattern separation could be negatively affected by transgenerational administration of a Western diet.

The researchers concluded that spatial pattern separation can help detect the effects of dietary interventions and that the Western diet can impair pattern separation.

Because of this, researchers hypothesized that apple extracts might exert beneficial effects on ASCs. They obtained apple extracts using ethanol as the extraction solvent and tested these on human adipose tissue-derived mesenchymal stem cells (ADSCs) and human cord blood-derived mesenchymal stem cells (CB-MSCs). They also used 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide and Click-iT 5-ethynyl-2?-deoxyuridine flow cytometry assays to evaluate the pro-proliferative effects of the extracts.

The researchers found that treatment with apple extracts promoted the proliferation of ADSCs and CB-MSCs. Apple extracts also induced the stepwise phosphorylation of p44/42 MAPK (ERK), mammalian target of rapamycin (mTOR), p70 S6 kinase (p70S6K), S6 ribosomal protein (S6RP), eukaryotic initiation factor (eIF) 4B, and eIF4E in ADSCs. p44/42 MAPK (ERK) is a signaling pathway involved in the regulation of cell proliferation and differentiation. Inhibition of this pathway results in cell apoptosis. mTOR is a key signaling node that coordinates cell cycle progression and cell growth. p70S6K is a cytokine that regulates cell growth by inducing protein synthesis. eIFs, on the other hand, are proteins or protein complexes involved in translation and protein biosynthesis.

The researchers also reported that apple extracts significantly induced the production of vascular endothelial growth factor (VEGF) and interleukin-6 (IL-6) in both ADSCs and CB-MSCs. VEGF is a potent angiogenic factor, which means it promotes the formation of blood vessels. VEGF also plays a role in other physiological functions, such as hematopoiesis, wound healing, and development. IL-6 is a promoter of proliferation. The researchers further confirmed that the apple extract-induced proliferation of ADSCs under serum-free conditions is mediated by ERK-dependent cytokine production because when they pre-treated cells with PD98059, a specific ERK inhibitor, it inhibited the phosphorylation of the mTOR/p70S6K/S6RP/eIF4B/eIF4E pathway.

Based on these findings, the researchers concluded that extracts from apples are potent pro-proliferative agents, and the beneficial effect of apple extract on the proliferation of ASCs may overcome the limitation in their therapeutic use in tissue regeneration.
